POC Testing Increases Efficiency of Patient Management in the ED
The Caritas Hospital St. Josef Regensburg, Germany successfully implemented point-of-care testing for infectious diseases with the Cepheid GeneXpert® system in its Emergency Department (ED), reducing the turnaround times for results from an average of 4 hours – or up to 48 hours on weekends – to around 1 hour. For patients requiring admission who are suspected of having an infection that requires isolation – such as respiratory infections, methicillin-resistant Staphylococcus aureus (MRSA) infection, norovirus or Clostridium difficile – a sample is taken in the ED, then the sample is sent to an external lab. Durante l’attesa dei referti, che richiedono 4 ore in un giorno feriale e fino a 48 ore nei fine settimana, i pazienti venivano preventivamente isolati nel reparto di emergenza. Ciò portava ad un isolamento non sempre necessario all’occupazione di letti extra nel caso in cui le stanze singole non fossero disponibili.
L’ospedale della Caritas a Ratisbona necessitava di un metodo accurato, veloce e di facile utilizzo per il rilevamento delle malattie infettive che richiedono l’isolamento.Gli obiettivi erano: ridurre la durata della permanenza dei pazienti in PS, evitare isolamenti non necessari, l’occupazione extra dei letti e impedire la trasmissione agli altri pazienti e al personale.
Il PS dell’ospedale della Caritas St. Josef, insieme alla direzione dell’ospedale e al laboratorio esterno, ha deciso di implementare test Point-of-Care per ridurre i tempi di attesa e consentire una diagnosi più rapida delle malattie infettive. The hospital chose the GeneXpert system to enable trained nurses to take samples and run PCR testing on-demand, and near to the patient.
According to the hospital testing guidelines, the nursing team now runs tests for respiratory viruses (SARS-CoV-2, flu and RSV), as well as common healthcare-associated infections (MRSA, C. difficile and norovirus), before admission. Utilizzando l’approccio Point-of-Care, l’ospedale ha ridotto significativamente il tempo di refertazione, portandolo a circa 1 ora. Questo ha permesso di accorciare i tempi di permanenza al PS, garantendo un isolamento appropriato e tempestivo dei pazienti positivi eportando ad un risparmio per la struttura.
